Scheduling appointments
Appointments can be scheduled during our normal business hours by calling 419-756-2122. We attempt to accommodate individual requests. We strongly suggest that you call as far in advance as possible for an appointment most convenient to your schedule.
On your first visit we ask that you arrive thirty minutes early so our staff can collect medical and insurance information from you. Please bring all your insurance cards with you. It is also a good idea to bring a list of current medications.
We are happy to accept new patients and will be glad to provide care for friends and relatives who are visiting the area.
Cancellations
If you are unable to keep your appointment, please notify us as soon as possible. This allows us to accommodate other patients.
Telephone Consultations
We will discuss individual patient cases over the telephone when possible. This includes questions regarding prescriptions, a particular treatment or symptoms. We ask that you limit telephone consultations to those situations when an office visit is not possible. Depending on the nature and complexity of the consultation, there may be a charge.
Confidentiality
Confidentiality of medical records is of the utmost importance to us. We ask that you provide a list of any individual with whom you will permit to discuss your case. Unless a person is specifically named, please respect our refusal to discuss any aspect of your case.
Billing
As a courtesy, we will file an insurance claim on your behalf. We attempt to file each claim timely, accurately and completely. Ultimately, the responsibility for the claim is with the patient. For those patients with no insurance, payment in full is expected on the date of service. We will provide a fee schedule upon your request. In addition, all co-pays and known deductibles must be paid on the date of service.
Any claim not paid by the insurance company 90 days from the date of service is considered past due, and must be paid in full. We will provide you with an insurance claim so that you may follow up with your insurance company.
